Complete all information under APPLICANT section and NEW DISCOVERY section,  part V

Minimum required attachments:

Production plat with 1 mile (oil) or 2 mile (gas) radius circle drawn from well.  All wells within circle should be labeled with spud date and producing formation(s).  For a category referring to a "producing interval" within a formation, to be considered, then log sections marking perforated intervals must be submitted for each well that produces from the same formation.
