INSTRUCTIONS FOR PREPARING FORM 1535

Check Box and Fill Out RESERVOIR DEWATERING PROJECT section and other sections that are applicable

Minimum required attachments:

A) Initial production test or data indicating 5:1 water/oil or water/gas ratio,
B) Structure and isopach maps of applicable reservoir, and                           
C) Schematic diagram of electrical grid and dewatering and water disposal  
equipment.
