Check Box and Fill Out ENHANCED RECOVERY PROJECT section and other sections that are applicable

Minimum required attachments:

A) A production test or data showing oil production indicates total content
of oil recovery after the use of enhanced recovery methods did not exceed
1% by volume,
B) Structure and isopach maps of applicable reservoir, and                         
C) Map and schematic diagram of producing, injection and disposal wells   
and other water injection or circulating equipment.
